Before your consultation, Dr. Bonza orders a comprehensive lab panel (billable through insurance) that goes far beyond standard bloodwork. Depending on your history and symptoms, this may include:

  • Metabolic assessment — CMP, lipid panel, hemoglobin A1c, fasting insulin, fasting glucose, high-sensitivity CRP, CBC, and more

  • Hormone & adrenal panel — estradiol, progesterone, testosterone, DHEA, cortisol, FSH, LH, SHBG, and full thyroid function (TSH, Free T3, Free T4, Reverse T3, TPO and thyroglobulin antibodies)

  • Nutritional assessment — vitamin D, B12, ferritin, iron studies, magnesium, folate, zinc, selenium, copper, homocysteine, and more

  • Genetic cancer screening (for those who qualify) — BRCA1/BRCA2, hereditary cancer panels, and tumor markers

Dr. Sarah Bonza, founder of Bonza Health, is a board-certified Family Physician, Certified Menopause Practitioner, and a trusted expert in women’s health.

Dr. Sarah Bonza is a board-certified Family Physician, Certified Menopause Practitioner (MSCP), Fellow of the American Academy of Family Physicians, and Diplomate of the American College of Lifestyle Medicine. She trained at The Ohio State University College of Medicine and has spent her career doing what most physicians don't: fully investigating the root causes of how women feel.

What's Actually Happening in Your Body.

The short answer is hormone fluctuations. But the full answer is more nuanced — and more important.

During perimenopause, which can begin as early as your mid-30s, estrogen, progesterone, testosterone, thyroid hormones, and cortisol can all shift in ways that affect nearly every system in your body. Sleep. Metabolism. Mood. Cognition. Weight. Energy. Libido. Bone density. Cardiovascular health.

These aren't separate problems. They're connected — and they deserve a connected, thorough medical response.

Unfortunately, most women are still being undertested and underserved when it comes to perimenopause. Symptoms are minimized, labs are incomplete, and women are sent home without real answers.

That's precisely why Dr. Bonza practices the way she does.

Here's what she commonly sees in her patients — and what your body may be trying to tell you:

  • This is arguably the most common “first” symptom that women experience when perimenopause arrives. You suddenly feel like you’re not yourself. Something inside you doesn’t “feel right,” and you feel like you’ve lost your luster or your love for life.

    You feel like you’re simply going through the motions of your day, or you don’t feel like doing anything you used to do.

    You feel disconnected with yourself, your relationships, your career, and your life; you can even feel lost about what to do next. 

  • Before perimenopause, your brain/mind felt sharp, clear, light, and efficient. You were able to do tasks quickly and thoroughly, remember even the smallest details, recall long-term memories, and get a good night’s sleep.

    Fluctuating hormones during the menopause transition can affect your cognitive functioning. Brain fog can be described in a wide variety of ways, and each woman can experience brain fog differently during perimenopause or menopause.

    This common symptom describes a set of cognitive-related experiences, including difficulty focusing/concentrating, forgetfulness, mental fatigue or sluggishness, inability to “find the right words” to say during a conversation, trouble retaining or recalling new information, and spacing out more often. 

  • Experiencing fatigue during perimenopause and menopause is due to several key hormonal changes: estrogen decline, progesterone decline, and adrenal fatigue.

    The difference between a day or two of feeling exhausted, sleep deprived or overworked and chronic fatigue caused by perimenopause or menopause is that this level of fatigue can be persistent and happen suddenly throughout the day, for weeks (or even months) on end.

    Menopausal fatigue can also cause your memory to falter, concentration challenges, and affect your overall quality of life. Even if you get a good night’s rest, you can still feel fatigued throughout the next day. 

  • When it comes to menopause, we all know about hot flashes and other common physical symptoms.

    But what’s important about this journey is that we focus on all aspects of your health, and that includes the mental and emotional sides as well.

    Because of significant hormonal changes, you can experience a wide range of mental- and emotional-related symptoms, such as mood swings, sudden moments of rage, intrusive thoughts, irrational fear, increased stress, anxiety, and depression. You can even be annoyed or irritated by things that never bothered you before.

    The drop in estrogen production (and other hormonal fluctuations) in the body during perimenopause influences your body’s regular functioning, including the biochemical elements that regulate mood and emotional well-being.

  • Sleep plays such a vital role to your hormonal health, and experiencing frequent sleep disturbances or waking up at 3 a.m. covered in sweat regularly can actually make your perimenopause symptoms worse.

    Symptoms experienced during perimenopause can be linked in a cause-and-effect cycle (e.g., lack of sleep due to hormonal changes leading to increased stress and mood swings, and vice versa), which can worsen symptoms if not managed properly. For example, lack of sleep can lead to fatigue, mood swings, low energy, and more stress and irritability; this can cause the famous menopause belly due to high cortisol levels.

    Feeling lost and not like yourself can also lead to depression. Some women may even experience insomnia during this time. The gradual decline of estrogen during perimenopause affects mitochondria functioning, energy levels, alertness, and focus while the decline in progesterone (also known as the “calming” hormone) can cause dysregulation in sleep cycles. 

  • If you’re experiencing changes to your weight and appetite, it’s a key sign that your hormones are changing due to perimenopause.

    Women can feel hungrier more often or experience intense cravings for sugary or salty foods. Your endocrine (hormonal production) system controls your hunger, but fluctuating hormones can create imbalances in your usual eating schedule and your diet.

    Estrogen decline during perimenopause can also make it challenging for women to maintain their weight. The reduced levels of estrogen production can also affect your insulin sensitivity and how your body stores fat (e.g., redistributing fat in the belly area or in the hips and thighs). High cortisol (stress) levels can also cause weight gain, especially around your midsection, causing the “belly bloat” or “menopause belly fat.”

    This is why strength training is highly recommended for women during this phase in their life. 

  • The main difference between perimenopause and menopause is your menstrual cycle: You can still have your period during perimenopause, but it can start becoming more irregular (this can vary a lot depending on each woman). Some women can experience heavier or even more painful periods.

    Your hormones are directly linked to your period, and it’s important to continue tracking your cycle during this time and the symptoms linked to it. 

  • During perimenopause, many women may not have hot flashes or night sweats, but it’s still worth noting that you can have these common menopausal transition symptoms.

    Hot flashes can occur suddenly during the day (frequency depends on each woman), and night sweats happen during your sleep, often waking you up and disrupting your sleep pattern.

    This can aggravate other symptoms you’re having, such as mood swings and energy. 

  • Other symptoms that are lesser known include stronger body odor, unexplained hair loss, joint pain or stiffness, urinary healthy issues, heart palpitations, skin changes (drier, more delicate or sensitive skin), tingling in extremities, and crawling skin sensation known as formication.

  • The Bonza Health Method is our comprehensive, year-long transformation program for women who are ready to commit fully to their health. It includes ongoing physician care, wellness coaching, strength training guidance, and personalized nutrition—addressing mind, body, and spirit. Because this is an intensive program, it's by application only. During your initial consultation, Dr. Bonza will assess whether you're a good candidate and discuss the details with you directly.

  • We currently offer in-person and telemedicine care for women in Ohio, and we provide telemedicine-only consultations and appointments for women in Michigan, Washington, and Kentucky. 

  • Bonza Health operates on a direct-pay model and does not accept insurance. However, we provide detailed Superbills that you can submit to your insurance for potential reimbursement. We also accept HSA and FSA payments.

  • Your primary care physician is managing 20+ different health conditions in 7-minute appointments. We focus exclusively on perimenopause and menopause optimization with 50-minute consultations, advanced testing, and individualized treatment plans. We have the time and specialized training to investigate deeply and optimize comprehensively to ensure you receive tailored care and longer visits. 

  • No, we do not. While hormone optimization is an option and often a cornerstone of treatment, we take a comprehensive, holistic approach that includes every part of your health, such as metabolic health, nutrition, lifestyle medicine, gut health, thyroid health, sleep, stress management, and much more. Your treatment plan will be centered around your unique needs and health goals.
